| You can register
with the Practice if you live within
our Practice area and are a
permanent UK resident.
Our Receptionist
will ask you to complete a
registration form and a medical
questionnaire. We also require photo
identification i.e.;
Passport/Driving license and proof
of address in the form of: Utility
bill, bank statement etc but not
mobile phone bill. If you have not
had a GP in the UK before you will
need to bring additional documents.
If you have been registered with the
NHS before, we will need your NHS
number. This will enable us to get
your medical records to the doctor
as quickly as possible.
We ask all new
patients to fill in a questionnaire
about their current state of health.
All patients aged 45 and over who
join the practice will be required
to attend an appointment with our
practice nurse where you will have
the chance to discuss your health.
We will also check your blood
pressure.
You
will be registered with the Practice
not an individual doctor but you may
choose which doctor you see each
time you make an appointment.
FACILITIES FOR OUR
DISABLED PATIENTS There is parking
at the surgery and an access ramp if
required.
We have WC
facilities suitable for patients
with disabilities.
