Our Receptionist will ask you to complete a registration form and a medical questionnaire. If you have your NHS medical card please bring this along. This contains useful information which will enable us to get your medical records to the doctor as quickly as possible.

We ask all new patients to fill in a questionnaire about their current state of health. All patients aged 45 and over who join the practice will be required to attend an appointment with our practice nurse where you will have the chance to discuss your health. We will also check your blood pressure.

You will be registered with the Practice not an individual doctor but you may choose which doctor you see each time you make an appointment.

FACILITIES FOR OUR DISABLED PATIENTS

There is parking at the surgery and an access ramp if required.

We have WC facilities suitable for patients with disabilities.
