NHS Health Checks are now being offered to people aged between 40 and 74 once every five years.
The check is to assess your risk of developing heart disease, stroke, kidney disease, diabetes or dementia. If there are any warning signs, then together we can do something about it.
By taking early action, you can improve your health and prevent the onset of these conditions. There is good evidence for this.
The Health Check involves a 15 minute appointment with our practice nurse or healthcare assistant. She will:
• Use straightforward questions and measurements such as age, family history, height, weight and blood pressure, to work out what risk factors you might have. She will then provide free personalised advice about what you can do to stay healthy.
• Take a blood test for blood sugar and cholesterol levels
Once your blood results are back, we will write to you with an overall assessment of your risk, including advice as to whether any further action is required
