CHANGES TO YOUR USUAL DOCTOR:  During the merger process of the last few months we have had to make a number of changes to the doctors' team.  This means we have had to move 10% of our patients to new usual doctors.  We are sending out text messages, emails and some letters to let you know if this applies to you.  If you have been moved, we are really sorry as we know patients and doctors often develop close relationships, and it can be frustrating having to get to know and to trust someone again.  However, please be patient and give it a go.  The new doctors we have are caring, conscientious and highly competent.

NHS England Care Data Update 21 February 2014

NHS England collects health and social care data via the Health & Social Care Information Centre (HSCIC).

We are asked to make you aware that certain limited patient information will be collected monthly from our records, commencing in spring 2014.

Your identity is protected by law, and the only information about you that will be collected is:

· NHS number

· Date of birth

· Postcode

· Gender

This will enable general practice data to be linked to hospital and community care data.

You may chose not to allow this data to be collected. If you do not want this data collected, then please let us know as soon as possible by completing the objection form below and we will note your choice on your medical records.
